    What will be shared: the story you write, your first name and last initial, your city and state, your likeness in any photos or video you submit, the condition or treatment you name, the approximate date of your care, and the fact that Dr. Hobbs was your surgeon. Nothing from your medical record is shared unless you write it in your story.

    Who is sharing it: Jonathan G. Hobbs, M.D. and Lakeshore Bone & Joint Institute.

    Who will receive it: anyone who visits doctorhobbs.com or the practice’s social media channels, and IndexMed, the platform the practice uses to receive and review submissions.

    Purpose: to share your experience with other patients and the public.

    Expiration: this authorization ends 10 years from the date you submit it, unless you revoke it sooner.

    Your rights: you may revoke this authorization at any time by calling (219) 250-5010 or emailing the office. Revocation takes effect going forward; it cannot recall what has already been published or copied by others. Your care, payment for care, and benefits do not depend on signing this. Once shared publicly, your information may no longer be protected by federal privacy law and could be re-disclosed by others.

    Please do not include other people’s names or details from your medical record. This form is not monitored for medical questions or emergencies; for those, call the office or 911.

    Who will see what I write?

    The practice team reviews every story first. Approved stories may appear on doctorhobbs.com and the practice’s social media with your first name, last initial, and city.

    Can I stay anonymous?

    We need your name and contact details to confirm your story is genuine, but only your first name and last initial are published. If you would rather use initials only, say so in your story and the team will honor it.

    Can I change my mind later?

    Yes. Call (219) 250-5010 or email the office and the story will be removed from the website and the practice’s channels. Copies already made by other people cannot be recalled.

    Is this form secure?

    Your submission is sent encrypted to the practice’s review system and is not stored on this website. Please keep other people’s names and details from your medical record out of your story. See the website privacy page.
